Healthy Living for ME® (HL4ME®) is Maine’s Community Care Hub (CCH), a collaborative effort involving local organizations, health systems, and volunteers. We aim to enhance health and wellness across Maine by coordinating community resources and empowering individuals to manage their health effectively, promoting a culture where prevention and wellness are prioritized. Our mission is to coordinate and align community resources to improve the health and wellness of the people of Maine. Our vision is to provide Maine people with the skills and resources to take control and build healthier lives.
Programs offered include education, fitness instruction, self-care strategies, health promotion activities, and social care coordination. HL4ME supports various needs such as chronic disease management, caregiver support, diabetes prevention, and intellectual/developmental disability support that address social determinants of health.
We are seeking volunteers to become class facilitators for our HL4ME workshops. Training is required and is provided at no cost to you. Workshops are held widely across our Area Agency on Aging counties dependent on the local area need. Volunteers must complete a volunteer application, be 18 years of age or older, and pass a background check. If you wish to teach a HL4ME workshop, please reach out as we would love to hear from you!
